Patient Satisfaction Survey

If you have seen one of our physicians in the office, we would love to hear from you about your experience. The survey is completely anonymous, unless you want us to contact you about it, in which case you will have an opportunity to provide us with your contact information at the end of the survey.

When completing the survey, please note that it is helpful for us to know the location you visited; please tell us in the last comment field.

To get started, click the name of the doctor you saw to be taken to the survey site for that physician, and think of your most recent visit with that doctor when answering the questions. If you were seen only in the infusion suite for your most recent visit and did not see a doctor, click on the link for Kane Hall Barry Infusion Only.

• Nerve Conduction Study - Read this if you are scheduled for an in-office EMG or NCV.

• Short Study EEG (1 hour) - Read this if you are scheduled for an in-office EEG.

• Ambulatory EEG (24-96 hour) - Read this if you are scheduled for an extended EEG.
